JOB SUMMARY
The Home Health Registered Nurse delivers high-quality, patient- and family-centered care in alignment with CCH's mission, vision, and values. Working closely with the Clinical Supervisor and Manager, this role coordinates and provides nursing care in the home setting, following the Wyoming Nurse Practice Act and ANA standards.
Key responsibilities include assessing patients, developing and implementing home care plans, administering medications, performing ordered tests and procedures, and documenting patient progress. The RN also promotes patient independence through education on conditions, medications, and self-care.
ESSENTIAL FUNCTIONS
- Provides specialized assessments, interventions, and education to patient and families tailored to end of life in Hospice.
- Provide skilled Home Health and Hospice nursing care in the patients home according to regulatory guidelines.
- Performs OASIS assessments proficiently.
- Performs patient assessment and documents in patient record.
- Develops an age-appropriate plan of care, which addresses physical, psychosocial and spiritual needs, in collaboration with other members of the health care team.
- Implements the plan of care, delegates appropriately and ensures documentation of interventions.
- Evaluates progress towards desired outcomes, revises plan of care and intervenes as necessary.
- Educates patient and families regarding current health status, plan of care, and anticipated outcomes.
- Maintains professional standards related to clinical practice, staffing and continuing education.
- Actively participates in hospital committees, CQI teams, and Quality Improvement Program.
- Maintains a clean and safe environment.
- Complies with the hospital's Corporate Compliance Program including, but not limited to, the Code of Conduct, laws and regulations, and hospital policies and procedures.
- Must be free from governmental sanctions involving health care and/or financial practices.
- Other duties as assigned. This list is non-exhaustive.
JOB QUALIFICATIONS
- Education
- Graduate of an accredited school of nursing.
- Licensure
- Wyoming RN license
- Certifications required
